The Rapid Re-Housing Case Manager for single adults is a grant-funded full-time, non-exempt position in the Emergency Housing Services Department.
As a critical part of the team, the Rapid Re-Housing Case Manager will provide the support services needed to ensure that program participants succeed in retaining their housing once services and financial supports are removed. Emphasis is placed on home visits and successful linkage to community support services needed to sustain permanent housing via increased self-sufficiency and quality of life.
Develop a Housing Sustainability Plan with each program participant.
Schedule regular home visits with services focusing on housing stability.
Assist program participants in accessing food, furniture, and ongoing utility assistance, as needed.
Coordinate appropriate linkages to available county and other mainstream services (ie mental health services, addiction recovery programs, and employment resources, as needed).
Provide transportation, as needed.
Maintain accurate and complete case notes for all program participants assigned to caseload.
Other related program tasks as assigned.
Qualifications: Bachelor's degree in social work, psychology or related field with a minimum of two years case management work experience.
